Fortcom Health Services is seeking a compassionate and skilled Per Diem Licensed Practical Nurse (LPN) to support both our Adult Family Care (AFC) and Critical Skilled Nursing (CSN) programs. This flexible, per diem role is ideal for an LPN who enjoys working independently, providing high-quality, home-based care, and collaborating with an interdisciplinary team.

The Per Diem LPN will conduct in-home visits and provide direct nursing care, support caregiver training, and complete clinical documentation for clients enrolled in our AFC and CSN programs. You’ll play a key role in ensuring that clients receive the care and services they need to remain safely in their homes, while also supporting caregiver compliance and program requirements.
Key Responsibilities:
- Conduct home visits to assess, monitor, and support the health needs of clients enrolled in the AFC and CSN programs.
- Provide direct nursing care based on individualized care plans and physician orders.
- Complete and coordinate all applicable clinical assessments and evaluations.
- Develop, implement, and review AFC plans of care in collaboration with the Program Director and nursing team.
- Train, evaluate, and supervise AFC caregivers to ensure quality and compliance.
- Monitor and document client conditions and report changes to the primary care provider or appropriate team members.
- Review physician summaries and ensure consistency with the plan of care.
- Deliver individualized, compassionate care in a home setting with a focus on comfort, safety, and dignity.
- Maintain accurate, timely, and compliant clinical documentation.
- Assist with additional office or clinical duties as assigned by the Program Director or COO.Â
Qualifications:
- Current and valid LPN license in good standing with the Massachusetts Board of Registration in Nursing.
- Minimum of 2 years of nursing experience; home health or community-based experience is a plus.
- Strong clinical judgment, time management, and documentation skills.
- Ability to work independently and communicate effectively with care teams and families.
- Proficient in Microsoft Word, Excel, and PowerPoint.
- Bilingual in Spanish or Haitian Creole is a plus.
- Must have reliable transportation to travel to member homes in assigned areas.

- Per diem position with flexible scheduling based on availability and caseload needs.Â
- Mileage reimbursement for all home visits.

Compensación: $35.00 per hour
