Responsibilities
The responsibilities of the Community Liaison include:
  • Assist in promoting the agency and fostering community relations
  • Apply unique understanding of the experience, language, or culture of the populations serve.
  • Assists in the evaluation of community needs and services to meet with identified needs.
  • Acts as liaison with member/caregiver and the agency
  • Establishes and maintains effective channels of communication.
  • Providing to the member and caregiver culturally appropriate health education, information, support, and counseling.
  • Participating in the development, implementation, and ongoing review of the AFC plan of care
  • Conducting on-site visits with the member at the qualified setting 
  • Assisting with obtaining information and accessing other health-care and community Services and making referrals to appropriate service provider s if the member requires services other than those provided by the AFC provider in conjunction with RN.
