- Assist in promoting the agency and fostering community relations
- Apply unique understanding of the experience, language, or culture of the populations serve.
- Assists in the evaluation of community needs and services to meet with identified needs.
- Acts as liaison with member/caregiver and the agency
- Establishes and maintains effective channels of communication.
- Providing to the member and caregiver culturally appropriate health education, information, support, and counseling.
- Participating in the development, implementation, and ongoing review of the AFC plan of care
- Conducting on-site visits with the member at the qualified setting
- Assisting with obtaining information and accessing other health-care and community Services and making referrals to appropriate service provider s if the member requires services other than those provided by the AFC provider in conjunction with RN.
